This project aims to optimise an evidence-based triage and navigation model of care into primary care practice.
Link-Me+ will use participatory design principles to identify the barriers and facilitators to create a person-centred model of care for triage and navigation of mental ill health, in the GP practice. It will draw on the expertise of those with lived experience, clinicians and policy makers to optimize, translate, implement and evaluate the model at scale.
